Masks Are ‘One of the Most Powerful Weapons We Have to Slow and Stop the Spread of the Virus,’ Says CDC Director

Americans are increasingly adopting the use of cloth face masks to slow the spread of COVID-19, and the latest science may convince even more to do so.

In an editorial published [yesterday] in the Journal of the American Medical Association (JAMA), CDC reviewed the latest science and affirms that cloth face coverings are a critical tool in the fight against COVID-19 that could reduce the spread of the disease, particularly when used universally within communities. There is increasing evidence that cloth face coverings help prevent people who have COVID-19 from spreading the virus to others.

“We are not defenseless against COVID-19,” said CDC Director Dr. Robert R. Redfield. “Cloth face coverings are one of the most powerful weapons we have to slow and stop the spread of the virus – particularly when used universally within a community setting. All Americans have a responsibility to protect themselves, their families, and their communities.”

This review included two case studies out today, one from JAMA, showing that adherence to universal masking policies reduced SARS-CoV-2 transmission within a Boston hospital system, and one from CDC’s Morbidity and Mortality Weekly Report (MMWR), showing that wearing a mask prevented the spread of infection from two hair stylists to their customers in Missouri.

Additional data in today’s MMWR showed that immediately after the White House Coronavirus Task Force and CDC advised Americans to wear cloth face coverings when leaving home, the proportion of U.S. adults who chose to do so increased, with 3 in 4 reporting they had adopted the recommendation in a national internet survey.

The results of the Missouri case study provide further evidence on the benefits of wearing a cloth face covering. The investigation focused on two hair stylists — infected with and having symptoms of COVID-19 — whose salon policy followed a local ordinance requiring cloth face coverings for all employees and patrons. The investigators found that none of the stylists’ 139 clients or secondary contacts became ill, and all 67 clients who volunteered to be tested showed no sign of infection.

The finding adds to a growing body of evidence that cloth face coverings provide source control – that is, they help prevent the person wearing the mask from spreading COVID-19 to others. The main protection individuals gain from masking occurs when others in their communities also wear face coverings.

COVID-19 prevention in a Missouri hair salon

When two stylists at a Missouri hair salon tested positive for the virus that causes COVID-19, researchers from CoxHealth hospitals, Washington University, the University of Kansas, and the Springfield-Greene County Health Department worked together to trace contacts, investigate the cases, and publish their findings in the MMWR.

One of the stylists developed respiratory symptoms but continued to see clients for eight days. The other, who apparently became infected from her co-worker, also developed respiratory symptoms and continued to see clients for four days.

The salon in which they worked had a policy requiring both stylists and their clients to wear face coverings, consistent with the local government ordinance. Both stylists wore double-layered cloth face coverings or surgical masks when seeing clients. The median appointment time was 15 minutes and ranged from 15 to 45 minutes. More than 98% of clients wore a face covering—47% wore cloth face coverings, 46% wore surgical masks, and about 5% wore N-95 respirators.

When customers were asked whether they had been ill with any respiratory symptoms in the 90 days preceding their appointment, 87 (84%) reported that they had not. None of the interviewed customers developed symptoms of illness. Among 67 (48%) customers who volunteered to be tested, all 67 tested negative for the virus that causes COVID-19. Several family members of one of the stylist’s subsequently developed symptoms and received a diagnosis of COVID-19.

Survey: Acceptance of face-mask guidance increased

CDC analyzed data from an internet survey of a national sample of 503 adults during April 7–9 and found that about 62% said they would follow the newly announced recommendations to wear a face mask when outside the home. A repeat survey during May 11-13 showed that the percentage of adults endorsing face mask wearing increased to more than 76%.

The increase was driven largely by a significant jump in approval by white, non-Hispanic adults, from 54% to 75%.  Approval among Black, non-Hispanic adults went up from 74% to 82%, and remained stable among Hispanic/Latino adults at 76% and 77%.

There was also a large increase in face-mask approval among respondents in the Midwest, from 44% to 74%.  Approval was greatest in the Northeast, going from 77% to 87%.

Anecdotes aren’t science. The Missouri hair salon reference is meaningless.

“Seriously people—STOP BUYING MASKS!” Surgeon General Jerome Adams tweeted on February 29. “They are NOT effective in preventing [the] general public from catching #Coronavirus, but if healthcare providers can’t get them to care for sick patients, it puts them and our communities at risk!”

If nonsurgical mask wearing to prevent viral spread was settled science it would have been settled long ago.

The CDC for the last link. But the real issue should be the misuse of studies to make absolute statements that are not really to the point. Contamination when in a hospital setting is not really relevant to casual use by the public. Nor is there any perfect solution. But as one medical commenter said, masks are not totally effective but they help. Social distancing is not totally effective but it helps. Cleaning surfaces is not totally effective but it helps. Add all these together and it makes a difference.

“For healthcare workers, there is evidence of efficacy of respirators if worn continually during a shift, but no evidence of efficacy of a mask (MacIntyre et al., 2011, MacIntyre et al., 2013).”

Further,

“A trial we conducted in Vietnam of 2-layered cotton cloth masks compared to medical masks showed a lower rate of infection in the medical mask group, and a 13 times higher risk of infection in the cloth mask arm (MacIntyre et al., 2015). The study suggests cloth masks may increase the risk of infection (MacIntyre et al., 2015), but may not be generalizable to all homemade masks. The material, design and adequacy of washing of cloth masks may have been a factor (Macintyre et al., 2020). There are no other randomised controlled trial of cloth masks published at this time, but if any protection is offered by these it would be less than even a medical mask.”

As to why consistency matters when ordering people around. Hating on one group of people while excusing another. Being hateful and rigid on one stance then turning 180 degrees and being just as hateful and rigid in the other direction discourages people from going on trust. Who knows what the next turnaround will be in people with a history of rudely dismissing other people’s concerns just because they don’t share them?

Protocol deviations were common during both EVD and CP PPE doffing, and some deviations during EVD PPE doffing were committed by the HCWs’ doffing assistant and/or trained observer. Self-contamination was common. PPE donning/doffing are complex and deserve additional study”

I can understand resenting government diddling with people’s activities whokesale with great abandon. But I can’t understand why face masks are such a big deal in light of the the much worse that the authoritarians and their sycophants have done. It is a Pyrrhic victory to rebel at something that might actually help a bit and costs so little to do, even if it is a straw clutched by the fearful, unless there is more to it than you do not think it of use and resent being forced to comply with something you consider silly.

Even among trained health care works the chance of self-contamination is “probable”. People who are not trained or used to dealing with a mask may be as likely to infect themselves with contaminated hands from adjusting and fiddling… and, perhaps, accumulation of virus on a masks surface.

My second point of consideration is the actual goal of policy. Four months ago it was “flatten the curve”… what is it now? It appears that it will be as common as the cold and we, as a population, need to deal with that. The current appoletic reaction to more infections is irrational against that reality.

I often value your commentary Ullr, even if I don’t agree. I am going to continue to wear a mask. And urge others to do the same.. to help protect the vulnerable. Masks alone aren’t 100% effective. Washing hands, not touching your face.. and social distancing are equally important.

Do you really mean to imply that the whole world can make the pandemic disappear if they just wear masks? That is denial too. Indonesia for examples a mandatory mask order in early April and their rate of infection has climbed steady at the same rate. To effectively reduce rates of infection a whole series of things need to be done, among which mask wearing is far down the list.

The problem at a basic, scientific level is that we can’t ask enough people to wear a mask (or not) and then intentionally expose them to various mechanisms of viral transmission and see who gets sick and who doesn’t in a randomized controlled setting to get effective statistics for each type of viral infection. So we resort to surrogates like the studies cited. Some are case studies, with all the shortcomings those have, while others attempt to measure other indices that suggest whether they work or not.

If the virus is largely transmitted through droplets emitted through coughing, sneezing, talking, etc. then mechanistic studies of cloth masks suggest they are effective in reducing the distance the droplets travel. If instead the transmission is primarily through persistent micro-aerosols with viral particles, then the masks will likely have only limited effectiveness to the point where it isn’t statistically significant.

Masks can reduce the inhalation of particles but cloth is pretty limited in that respect, ~10-20% according to the studies I saw as compared to the N95 which reduce small particle aerosols by 95%. The problem with masks is that if you constantly touch contaminated surfaces and then constantly adjust your mask you can increase your exposure by bringing viral particles to your face repeatedly. That was the case in one of the studies cited above.

So to summarize, the actual hard science about masks is still fairly limited but the case studies combined with the mechanistic studies suggest they can be largely effective at reducing aerosol transmission. It’s the best we have to go on at the moment though I don’t doubt there will be other studies coming out of the current event in the coming months and years.

I’m offering information for critical assessment because I don’t think the general population should be treated as if we are all toddlers. People, given the best information available, will act in their best interest. A certain percentage of people, being told what to do by faceless bureacracies, are going to tell ’em to fuck off. This is an interesting study. Never mind the hairdressers went to work sick.. that’s not cool. But it’s a good example of how masks are actually effective. The thing that irks me is how the CDC and others discredit masks from the beginning. Obviously masks cut transmission, for them take such statements about not having masks, simply because of supply issues, endangered people. I don’t necessarily agree with mandatory masking. I know I hate mine. But wear one anyways..

If they are claiming that all those studies are no longer valid but the new ones are why should we put any trust in them?

This is the crux of the study. Basically that we didn’t have a good enough set of trials nor a good enough understanding of specific viral transmission pathways to be definitive about the effectiveness of masks in every case. However, based on the best current understanding of how this virus is transmitted, the mechanistic studies suggest masking will be effective in reducing viral transmission.

It’s been months since I’ve seen anyone not wearing a mask.
Maybe because all businesses require it and most people respect
the businesses they go to.

At the same time the left with their kill grandma rant gets old very quickly.
We all have one thing in common we care about our family and friends.

The constant nagging about masks is doing it’s job of dividing people and causing more stress for political reasons.

WHO,CDC,NIH etc. have studied viruses for decades, I expected them to actually have a plan not a constant stream of errors.
I think they have forfeited all trust.

Local info. follows what ever the CDC etc. tell them to say.

Gavin should also know we are not orange county! or SF.
and react accordingly.

Since I watch what goes on I bought 100 masks before everyone ran out of them in January while Nancy was pushing trips to Chinatown..
